Do you complete disability forms?
Yes, please allow 7-10 business days to complete disability and lost-time forms. The first form is free, however, there is a $15.00 fee for each additional form.
There is not a charge for Ohio Bureau of Worker's Compensation Forms. Please allow 5-7 business days for completion of these forms.

How are payments arranged?
We bill only for procedures performed by the physician and their assistant (their charges are separate from your other hospital bills). If we are not contracted with your insurance company, please make arrangements to pay at the time of service. We accept cash, money order, VISA, Mastercard, Discover, American Express and CareCredit. Please feel free to inquiry about our charges. If necessary you can make payment arrangements through our Billing Office at 937-208-5392.

When can I get my prescription refilled?
Please call our office between 9:00am and 5:00pm Monday through Friday to request medications or prescription refills. Please have your pharmacy name and phone number, name of medication, and refill number before calling. Refills will NOT be renewed after office hours, so please do not wait until you run out before calling.
